 Ziprasidone and affective switching

Title: Ziprasidone-associated mania: a case series and review of the mechanism.
Reference: Bipolar Disorders 2003;5:72-5.
Purpose: To discuss the clinical and pharmacological evidence that ziprasidone may cause affective switching.
Study design: Retrospective case series.
Follow up: Variable.
Patients: 4 selected bipolar cases.
Treatment: Ziprasidone 20–100 mg/day added to other medications.
Results: Four cases were described in which mania occurred following initiation of ziprasidone therapy. The authors hypothesize that the substantial serotonergic and noradrenergic actions of ziprasidone may make affective switching more likely to occur with ziprasidone compared with other atypical antipsychotics.
